History of Falls

• Fill the appropriate value in the respective fields.
• By default there is 3 History of Falls form. You can add other by clicking Add another History of fall button.
• After adding another form, a Remove key pops up clicking Remove you can delete recently added form from the list.
• After filling all the fields you can Save Save and add another Save and continue editing
