History of Falls

* Fill the appropriate value in the respective fields.
* By default there is 3 :ref:`history-of-falls` form. You can add other by clicking **Add another History of fall**
  button.
* After adding another form, a *Remove* key pops up
  clicking *Remove* you can delete recently added form from the list.

* After filling all the fields you can **Save** **Save and add another** **Save and continue editing**
