Fall Risk Assessment Tool

• Fill the appropriate value in the respective fields or fill from the drop down menu.
• By default there is 3 charts of the resident. If you want to add more chart then select Add another History of fall which is at the bottom of the page.
• After adding another chart, a Remove key pops up clicking Delete you can delete recently added chart from the list.
• After filling all the fields you can Save Save and add another Save and continue editing
