Fall Risk Assessment Tool

* Fill the appropriate value in the respective fields or fill from the drop down menu.


* By default there is 3 charts of the resident. If you want to add
  more chart then select **Add another History of fall** which is at the bottom of the page.
* After adding another chart, a *Remove* key pops up
  clicking *Delete* you can delete recently added chart from the list.

* After filling all the fields you can **Save** **Save and add another** **Save and continue editing**
