  • Fill the appropriate value in the respective fields.
    • Select either yes or no in Yes/N0 field from the drop down menu.
    • Give the appropriate comment in the Comment section.
    • Click Choose file to upload signature in the Assessors signature field.
    • Enter date in the Date field manually or select from the Calender icon to the right.
  • After filling all the fields you can Save Save and add another Save and continue editing
  • By default there is 1 Bed Rails Risk Assessment form. You can add other by clicking Add another Bed rails button.
  • After adding another form, a Remove key pops up clicking Delete you can delete recently added form from the list.
