Bed Rails Risk Assessment

* Fill the appropriate value in the respective fields.
    * Select either yes or no in **Yes/N0** field from the drop down menu.
    * Give the appropriate comment in the **Comment** section.
    * Click **Choose file** to upload signature in the **Assessors signature** field.
    * Enter date in the **Date** field manually or select from the Calender icon to the right.

* After filling all the fields you can **Save** **Save and add another** **Save and continue editing**


* By default there is 1 :ref:`bed-rails-risk-assessment` form. You can add other by clicking **Add another Bed rails**
  button.
* After adding another form, a *Remove* key pops up
  clicking *Delete* you can delete recently added form from the list.
