This section contains the following fields to be filled up:

  • Enter the date in the Date field.
  • Enter the name in the Next of Kin Name field.
  • Give telephone number in Telephone Number section.

Note

Scroll the scroll bar to add other details

  • Enter the name in the STAFF CALLING PRINT NAME
  • Enter the brief information in the BRIEF OF INFORMATION GIVEN field.
  • If you want to add more fields then select Add another Next of kin contact which is at the bottom of the page.
  • After adding fields, a Remove key pops up clicking Remove you can delete recently added chart from the list.

After filling all the fields you can Save Save and add another Save and continue editing
