Choking Risk Assessement

* Fill the appropriate value in the respective fields.
* Enter date in the **Date** field manually or select from the Calender icon to the right.
* In the **CHECKLIST OF INDICATORS / OBSERVATIONS** fields select the appropriate value from the drop down menu.

* After filling all the fields you can **Save** **Save and add another** **Save and continue editing**
