Dementia Dependency Assessment Tool

* Each drop down field contains the option to choose with certain score of each.
* Sub total of each category are calculated in the Sub total field.
* Enter the date manually or select from the date icon to the right in every date field.
* You can add another Dementia Dependency form by clicking Add another Dementia. After adding new Dementia Dependency Form you can remove it by clicking Remove button.
* After filling all the fields you can Save Save and add another Save and continue editing
